ISLAND (NASSAU AND SUFFOLK COUNTIES)
SUPPORTED HOUSING PROGRAMS
MTI’s overall purpose in its
Supported Housing Programs is to
enhance the quality of life of the
tenant members who are chronically
homeless single adults with a
serious mental illness and who may
have co-occurring substance abuse
disorder. The Supported Housing
program assists individuals to
bridge mainstream community life
with coordinated support systems of
care. The programs provide permanent
housing; address any challenges,
i.e. educational, social, health,
etc. and when appropriate, the
program provides community-life
skills training in a "place and
train" model; pro-active
consultation, monitoring, role
modeling and, hands on supports as
needed.
MTI prioritizes admission for all
adult residential referrals from
priority populations as defined by
the New York State Office of Mental
Health. MTI will house individuals
with serious mental illness may have
co-occurring substance abuse
disorders and are members of the
following OMH’s priority
populations:

Individuals discharged directly from
a state psychiatric centers;

Individuals from state operated
residential programs (congregates
treatment or congregate support
facilities);

Individuals from Article 28
inpatient hospital units;
-
Individuals from the streets or
homeless shelters;
-
Residents moving from adult homes;
-
Children’s Residential Treatment
Facilities (youth who are aging out
of the system).

Individuals under an Assisted
Outpatient Treatment (AOT) Court
Order;
-
An individual who moves from other
OMH licensed or funded housing
programs to supported
housing, the individual
moving to supported housing will be
considered a priority admission if
the vacancy created by this move is
filled with an individual meeting
the priority admission criteria
listed above.
ALL OF THE SUPPORTED HOUSING
PROGRAMS IN
LONG ISLANDARE for
people with mental illness who are
able to live independently with
minimal support services. Residents
should be capable of seeking
assistance if necessary. Residents
generally must be able to take their
medication independently and
demonstrate a significant period of
psychiatric stability. Overall,
residents should be able to meet
their own daily needs and manage
their household with minimal
assistance from staff. The minimum
age for applicants is 18.
MTI currently accepts and places
ALL referrals through the Nassau
County Single Point of Access (SPA)
process. Please complete (SPA
Application) and submit the
following documents:
- Psychiatric Evaluation (w/in 1
year)
- Psychosocial Evaluation (w/in 1
year)
- Physical (w/ in 1 year)
- PPD (w/ in 1 year)
